Quality of Life Scale (HHHHHMM)
This tool is designed to help you see the trends in your senior pet’s health. Be honest with yourself—it’s the kindest thing you can do.
Note: 0 = Very Poor, 10 = Excellent/Normal
1. Hurt & Pain Management
Is your pet breathing easily? Is pain managed successfully? (0 = In pain/panting, 10 = Comfortable)
2. Hunger
Is your pet eating enough? Do they need hand-feeding? (0 = Refuses food, 10 = Eats normally)
3. Hydration
Are they drinking enough water? Are they dehydrated? (0 = Dehydrated/needs fluids, 10 = Drinks normally)
4. Hygiene
Can they keep themselves clean? Do they have accidents or sores? (0 = Cannot groom/soiled, 10 = Clean/groomed)
5. Happiness
Is there joy? Do they greet you? Do they have “spark”? (0 = Depressed/anxious, 10 = Happy/engaged)
6. Mobility
Can they get up and move around without help? (0 = Cannot stand, 10 = Active/Mobile)
7. More Good Days Than Bad
When you look at the last week, does the good outweigh the bad? (0 = Bad days rule, 10 = Mostly good days)
